Four Marks & Medstead ward lies in the eastern part of the district, characterised by its rural landscape of farmland, woodland, and scattered settlements. The area includes the linear village of Four Marks, which developed around the old A31, and the more historic village of Medstead to the north. The terrain is gently rolling, with fields and hedgerows defining much of the scenery.

The Alton Line railway runs through the ward, connecting it to Alton and Winchester, with a station at Medstead and Four Marks. Local land use is primarily agricultural, with some small-scale horticulture. The Watercress Line heritage railway passes just to the north, its steam trains occasionally visible from parts of the ward, adding a distinctive sight and sound to the environment.
